If you are sending in more than one pager, fill out a form for each pager you ship.
This only ensures that the process of returning the pager to you will be faster.

When shipping:
1.) Turn pager off before shipping.
2.) Pack pager in a secure box, Do not ship in an envelope.
3.) ENCLOSE A COMPLETED SINGLE OR MULTIPLE REPAIR FORM IN THE BOX.
4.) If there is no repair form or paper saying the reason it was shipped, then: The pager if ours, will be kept until we hear from you. The pager if yours will be sent back with no repair done.
5.) When calling on status, Please refer to the tag number (assigned by us), Pager number, or Serial number
